سلامت دهان و دندان از مسایل مهم برای مردم و پزشکان بوده و شناسایی عوامل موثر در آسیب به سلامت دهان و دندان بسیار مهم است. یکی از عواملی که در آسیب رساندن به سلامت دهان و دندان همواره بر آن تاکید میشود، اعتیاد به مواد مخدر است. هدف اصلی در این پژوهش، مرور نظامند وضعیت سلامت دهان در مصرفکنندگان مواد مخدر است.
روش کاراین مطالعه یک مرور نظاممند از نوع متاآنالیز (فراتحلیل) است. در مقالات فارسی و انگلیسی توصیفی مقطعی که مقایسه شاخصهای سلامت دهان و دندان در افراد مبتلا به سوء مصرف مواد با افراد سالم را بررسی کرده بودند از پایگاههای الکترونیکی پابمد، گوگل اسکولار، اسکوپوس، امبیس و ISI جستوجو شد. کیفیت مطالعات انتخاب شده توسط مقیاس نیوکاسل-اوتاوا سنجیده شد. ناهمگنی مطالعات با آزمون کیو و شاخص آی اسکویر بررسی شد. در صورت ناهمگنی مطالعات، از مدل اثرات تصادفی (random effect model) برای ترکیب نتایج استفاده شد. سوگرایی نشر توسط منحنیهای قیفی و بررسی تستهای اگر (Egger) و بگ (Begg) ارزیابی شد.
یافتهها:
سوء مصرف مواد مخدر در مقایسه با افراد سالم یک اثر معنیدار روی شاخص DMFT نشان میدهد، طوری که مقدار میانگین برآورد شده برای کل مطالعات بیشتر است. (P<0/001; stddif in min 1/657[1.873-1/442. همچنین پلاک ایندکس در افراد مبتلا به سوء مصرف مواد مخدر در مقایسه با افراد سالم بیشتر است (OR. 1/42; 95% CI 1/18-1/7) P = 0/0002).
نتیجهگیری:
پس از تجزیه و تحلیل مشخص شد که میانگین DMFT در افراد مبتلا به سوء مصرف مواد مخدر در مقایسه با افراد سالم بیشتر است. همچنین مشکلات پریودنتال در افراد مبتلا به سوء مصرف مواد مخدر در مقایسه با افراد سالم بیشتر بوده و بنابراین وضعیت بهداشت دهان و دندان این افراد نیازمند توجه بیشتر است.
پیامدهایعملی:
سازمانهای متولی و دندانپزشکان اجتماعی با رویکردی جدیتر نسبت به بهداشت و مشکلات دهان و دندان افراد مبتلا به سوء مصرف مواد مخدر بیاندیشند.

تصادفات جاده ای یکی از علل اساسی مرگ و میر به شمار میآید و بررسی عوامل موثر بر آن امری حیاتی است. بر اساس مطالعات انجام شده، رانندگان مبتلا به اختلال توجه به دلیل کمبود توجه، عملکرد رانندگی ضعیف تری نسبت به سایر افراد دارند و با توجه به الگوی رانندگی متمایز آنها، بررسی عملکرد رانندگی آنها در شرایط مختلف حایز اهمیت است.
روش کاردر این مطالعه روش شناختی 31 فرد مبتلا به اختلال نقص توجه-بیش فعالی و 31 فرد در گروه کنترل، 18 تا 59 سال از هر دو جنس، پس از تشخیص قطعی توسط روانپزشک به صورت تصادفی انجام آزمون تست وینا و دستگاه شبیه ساز رانندگی سهند به آزمایشگاه مرکز تحقیقات مدیریت و پیشگیری از مصدومیت های ترافیکی ارجاع داده می شوند. هدف این مطالعه مقایسه عملکرد رانندگی رانندگان مبتلا به اختلال نقص توجه-بیش فعالی با گروه کنترل و محاسبه ریسک خطر رخداد حوادث ترافیکی در این دو گروه است. علاوه بر این، بررسی مشترک نتایج دستگاه شبیهساز رانندگی و آزمون های سیستم تست وینا مانند: زمان واکنش و تست ادراک تطبیقی نیز انجام خواهد شد.

سلولهای گلیا، گلیوترانسمیترهای مختلفی رها میکنند و نیز به میانجیهای عصبی آزاد شده توسط نورونها پاسخ میدهند. سلولهای گلیا به خصوص آستروسیتها با داشتن ترانسپورترهای گوناگون، نقش مهمی در هومیوستاز فضای سیناپسی و شکلپذیری سیناپسی ایفا میکنند. در این مطالعه به بررسی نقش ترانسپورتر گلوتاماتی سلولهای گلیا (Excitatory amino acid transporter 2) هیپوکمپ یا EAAT2 بر پاسخ سیناپسی پایه و شکلپذیری سیناپسی کوتاه مدت پرداخته شده است.
روش کاراین مطالعه از نوع تجربی است. جهت فعالکردن ترانسپورتر گلوتاماتی سلولهای گلیای هیپوکمپ، 0/5 میکرولیتر از محلول 0/5 میلیمولار سفتریاکسون، فعالکننده این ترانسپورتر، به صورت دو طرفه در داخل هیپوکمپ موش صحرایی نر نژاد ویستار تزریق شد. پاسخ سیناپسی پایه و شکلپذیری سیناپسی کوتاه مدت با تکنیک ثبت پتانسیل میدانی بررسی شد. مسیر شاخه جانبی شافر تحریک و پتانسیل میدانی پس سیناپسی تحریکی (field excitatory postsynaptic potential) از CA1 ثبت شد. پس از تهیه نمودار Input/output، از تحریکات زوج پالس (Paired pulse) برای القای شکلپذیری سیناپسی کوتاه مدت استفاده شد.
یافته هافعالکردن ترانسپورتر گلوتاماتی سلولهای گلیای هیپوکمپ توسط ریز تزریق سفتریاکسون در ناحیه CA1، پاسخ سیناپسی پایه را متاثر نکرد (Two Way ANOVA, P>0/05). فعالکردن این سلولها تغییر معنیداری در شاخص زوج پالس در فاصله بین پالسی 20، 80 و 200 میلیثانیه ایجاد نکرد (Two Way ANOVA, P>0/05).
نتیجهگیریفعالکردن ترانسپورتر گلوتاماتی سلولهای گلیا هیپوکمپ بر پاسخ سیناپسی پایه و شکلپذیری سیناپسی کوتاه مدت در ناحیه CA1 هیپوکمپ تاثیری نداشت.
پیامدهایعملیبا توجه به نقش سلولهای گلیا در تنظیم تحریکپذیری سیستم عصبی و همچنین شکلپذیری سیناپسی، یافتن راهکارهایی جهت تصحیح این ویژگیهای سیستم عصبی با دستکاری سلولهای گلیا میتواند در درمان یا پیشگیری از بیماریهای سیستم عصبی کمککننده باشد. در این مطالعه به بررسی نقش سلولهای گلیا در هومیوستاز نوروترانسمیتر تحریکی گلوتامات در فضای سیناپسی هیپوکمپ، از طریق تحریک برداشت آن، بر فعالیت سیناپسی پایه و شکلپذیری سیناپسی کوتاه مدت پرداخته شد.

اختلال نقص توجه و بیشفعالی (ADHD) یکی از شایعترین اختلالات روانپزشکی با شروع در سن کودکی است که سردسته علایم آن بیش فعالی، نقص توجه و تکانشگری هستند. هدف این مطالعه بررسی تغییرات مولکولی ژن DRD4 و ارتباط آن با ADHD با توجه به دوز متیل فنیدیت بود.
روش کارجامعه آماری تحقیق شامل تمام افراد 5 تا 18 ساله مبتلا به ADHD شهر تبریز بود که به درمانگاه روانپزشکی بزرگمهر وابسته به دانشگاه علوم پزشکی تبریز مراجعه کرده و نیازمند درمان دارویی بودند. شدت علایم با استفاده از مقیاس کانرز ارزیابی شد. سپس 4 میلی لیتر خون از هرکدام از افراد گرفته شده و DNA با استفاده از روش Salting out استخراج شد. بعد از انجام PCR محصولات با استفاده از آنزیم های محدود کننده اختصاصی به قطعات مورد نظر تقسیم شده و بعد از ژل الکتروفورز در آگاروز 2 درصد، پلیمورفسیمهای Ex3 VNTR و rs3758653 ژن DRD4 بررسی گردید. آخرین دوز مصرفی برای رسیدن به پاسخ درمانی بر اساس میلیگرم بر کیلوگرم ثبت شد. سپس وجود رابطه بین دوز داروی مصرف شده برای دستیابی به پاسخ درمانی و وجود پلی مورفیسم های ذکر شده بررسی شد.
یافتههابین وجود پاسخ به درمان و نوع پلی مورفیسم رابطه معنی دار آماری دیده نشد. بین میانگین دوز دارو و پلی مورفیسم های 3 تکرار و 4 تکرار و 6 تکرار و بیش از 6 تکرار در پلی مورفیسم EX3VNTR در ژن DRD4 رابطه معنی دار آماری دیده نشد. بین میانگین دوز دارو و ژنوتیپ های CCو TCو TTدر پلیمورفیسم rs3758653 موجود در ژن DRD4رابطه معنی دار آماری دیده نشد.
نتیجهگیریبین وجود پاسخ به درمان و نوع پلی مورفیسم رابطه معنی دار آماری دیده نشد. بین میانگین دوز دارو و پلی مورفیسم های 3 تکرار و 4 تکرار و 6 تکرار و بیش از 6 تکرار در پلی مورفیسم EX3VNTR در ژن DRD4 رابطه معنی دار آماری دیده نشد. بین میانگین دوز دارو و ژنوتیپ های CCو TCو TTدر پلی مورفیسم rs3758653 موجود در ژن DRD4رابطه معنی دار آماری دیده نشد.
پیامدهای عملیتعیین میزان کاهش علایم ADHD با مصرف متیل فنیدیت و تعیین وجود یا عدم وجود پلی مورفیسم های DRD4 و بررسی رابطه بین دوز دارو برای رسیدن به پاسخ به درمان می تواند موجب جمع آوری شواهد کافی برای تعیین دوز متیل فندیت با توجه به آنالیز مولکولی ژن DRD4 در بیماران مبتلا به اختلال نقص توجه و بیشفعالی شود. این عامل می تواند باعث کم شدن مراجعهها به پزشک جهت تنظیم دوز دارو، صرفه جویی در زمان و کاهش تجربه عوارض جانبی آن دارو شود.

زمینه:
بیماری کووید-19 با قدرت انتقال و بیماریزایی بالا، عامل ابتلا و مورتالیته روزانه بسیاری از مردم سراسر جهان میباشد. تاکنون درمانهای مختلفی برای این بیماری بکارگرفتهشده که یکی از اینها تزریق ایمونوگلوبولین داخل وریدی (IVIg) است. این مطالعه با هدف تعیین اثر تزریق IVIg در درمان بیماران مبتلا به طوفان سیتوکاینی ناشی از کروناویروس بود.
روش کار :
صدوهفتادوچهار بیمار مبتلا به کووید-19 تاییدشده با مشخصات بالینی و یافتههای آزمایشگاهی در این مطالعه وارد و بر اساس دریافت ایمونوگلوبولین داخل وریدی به دو گروه دریافتکننده IVIg و غیردریافتکننده IVIg تقسیمشدند. تجزیه و تحلیل آماری با استفاده از SPSS نسخه 0/22 انجامگرفت و مقادیر P کمتر از 0/05 ازنظرآماری معنیدار درنظرگرفتهشد.
یافته هاIVIg تنها در درمان 20 بیمار استفادهشد و داروهای کالترا و هموپرفیوژن در بیماران دریافتکننده IVIg بیشتر مورداستفاده قرارگرفتند (به ترتیب P=0/003 و P=0/001). فراوانی تست PCR مثبت کووید-19 در گروه دریافتکننده IVIg بهطورمعنیداری بیشتر بود (P=0/026). تعداد نوتروفیل پلی مورفونوکلیار (PMN) (P=0/007) و سطح سدیم (P=0/007) در گروه دریافتکننده IVIg در روز اول پذیرش بهطورمعنیداری بیشتر بود. علاوه بر این، سطح نسبت نرمالشده بینالمللی (INR) در گروه دریافتکننده IVIg در روز هفتم بهطورمعنیداری کمتر بود (P=0./002). میانگین کل روزهای بستری در بخش مراقبتهای ویژه (ICU) در بیماران دریافتکننده IVIg بهطورمعنیداری بیشتر بود (P=0/001)
نتیجهگیریبه نظرمیرسد بکارگیری تزریق IVIg در بیماران کووید-19 بایستی مورد بررسی بیشتری قرار گیرد. پیامدهای عملی. تزریق IVIg توانست مورتالیته بیماران کووید-19 را کاهش و میزان بقا را به میزان کمی افزایش دهد.

مننژیت کودکان با عوارض و مرگومیر بسیاری همراه بوده و بنابراین توسعه روشهای مطمین و در دسترس برای پیشبینی این عوارض و انجام اقدامات زودرس جهت جلوگیری از پیشرفت عوارض و نیز درمان آنها بسیار ارزشمند است. هدف از مطالعه حاضر بررسی یافتههای سیتیاسکن در بیماران مبتلا به مننژیت و ارتباط آن با پیشآگهی بیماری در مرکز پزشکی کودکان تبریز بود.
روش کاردر مطالعه تحلیلی-مقطعی حاضر، پرونده 100 بیمار مبتلا به مننژیت (با اولویت مننژیت باکتریال) بین سالهای 93 تا 97 در بیمارستان کودکان بررسی شد. نتایج سیتیاسکن و عوارض بیماری ثبت شده و با استفاده از نرمافزارهای آماری تجزیه و تحلیل شد.
یافتههاعلایم شایع شناسایی شده در بیماران مورد مطالعه به ترتیب شامل تب (88 درصد)، تهوع و استفراغ (36 درصد)، سردرد (27 درصد) و تشنج (25 درصد) بود. همچنین نتایج تفاسیر سیتیاسکن شامل افیوژن مغزی (16 درصد)، هیدروسفالی (7 درصد)، ادم منتشر مغزی (7 درصد) و آتروفی مغزی (4 درصد) بود. بررسی های آماری نشان داد که میان عوارض مننژیت (اختلال یادگیری، صرع و اختلال شنوایی) و نتایج حاصل از تفسیر سیتیاسکن (افیوژن مغزی ادم منتشر مغزی و هیدروسفالی) ارتباط آماری معنیداری وجود دارد.
نتیجهگیریبررسی نتایج این مطالعه حاکی از آن بود که سیتیاسکن میتواند در کودکان مبتلا به مننژیت به عنوان روش تشخیصی زودهنگام و پیشآگهی استفاده شود.
پیامدهای عملیمننژیت با عوارض و مرگومیر بسیاری همراه بوده و عدم وجود روشهای تشخیصی و پیشآگهی دقیق از چالشهای مهم در این زمینه است. شناسایی و معرفی روشهای دقیق و ارزان برای پیشبینی این عوارض و انجام اقدامات زودرس جهت جلوگیری از پیشرفت عوارض و درمان بسیار سودمند است. نتایج مطالعه حاضر نشان داد که سیتی اسکن میتواند در کودکان مبتلا به مننژیت به عنوان روش تشخیصی زودهنگام و پیشآگهی استفاده شود.

تولد نوزاد نارس علاوه بر ایجاد مشکلات بهداشتی و سلامتی در نوزادان، باعث افزایش استرس مادر و کاهش دلبستگی بین مادر و نوزاد میشود. بنابراین، مطالعه حاضر با هدف تعیین تاثیر اجرای تجسم هدایت شده بر استرس و دلبستگی مادران نوزادان نارس در بخش مراقبت ویژه نوزادان انجام شد.
روش کاراین مطالعه نیمه تجربی روی 70 مادر دارای نوزاد نارس بستری در بخش مراقبتهای ویژه نوزادان انجام شد. مادران به روش نمونهگیری در دسترس انتخاب و به طور تصادفی در دو گروه مداخله و شاهد (هر گروه 35 نفر) وارد شدند. برای مادران گروه مداخله از روز اول تا پنجم تولد، آموزش از طریق ارسال فایلهای تجسم هدایت شده انجام شد. در روز پذیرش و ترخیص پرسشنامه استرس والدی و پرسشنامه دلبستگی بعد از تولد، توسط مادر تکمیل شد. جهت تجزیه و تحلیل داده ها از نرم افزار SPSS نسخه 20 استفاده شد.
یافتههایافتههای حاصل از این مطالعه حاکی از آن بود که میانگین کل نمره دلبستگی مادران در روز پنجم مداخله، در گروه مداخله بهطور معنیداری بالاتر بود (0/001>P). همچنین میانگین کل نمره وضعیت دلبستگی مادران روز ترخیص، در گروه مداخله به طور معنیداری بالاتر بود (0/001>P). میانگین نمره استرس مادران در روز پنجم مداخله و همچنین در روز ترخیص در گروه مداخله بهطور معنیداری پایینتر بود (0/001>P).
نتیجهگیری:
نتایج مطالعه نشان داد که اجرای برنامه تجسم هدایت شده باعث کاهش استرس در مادران دارای نوزاد نارس بستری در بخش مراقبتهای ویژه نوزادان شده و افزایش میزان دلبستگی در این مادران را در پی دارد.
پیامدهای عملی:
مادران مراقبان اصلی نوزادان هستند، بنابراین ارتقای سلامت مادران بر نوزادان تاثیر دارد. این مطالعه علاوه بر کاهش تنش مادران نوزادان نارس میتواند به بهبود کیفیت مراقبت از نوزاد و همچنین به افزایش دلبستگی و پذیرش نقش والدی کمک کند. مطالعه حاضر روشی کاملا مقرون به صرفه جهت کاهش هزینههای بستری طولانی مدت و ارتقای کیفیت پرستاری ارایه میکند.

مادران دارای کودک مبتلا به طیف اوتیسم فشار روانی و جسمانی زیادی را تحمل میکنند که میتواند بر کیفیت زندگی، حالات عاطفی و قضاوت آنها از خدمات درمانی دریافت شده تاثیر بگذارد. هدف این مطالعه مقایسه کیفیت زندگی، سطح اضطراب و میزان رضایتمندی مادران دارای کودک مبتلا به طیف اوتیسم با سایر مادران دارای کودک غیر اوتیستیک در شهر اهواز بود.
روش کاردر این مطالعه مقطعی، توصیفی-تحلیلی از بین مراجعین مراکز توانبخشی اهواز، 128 مادر دارای کودک مبتلا به طیف اوتیسم و 30 مادر دارای کودک نیازمند مداخلات توانبخشی غیر اوتیستیک انتخاب شدند. پرسشنامه کیفیت زندگی، مقیاس اضطراب درک شده-14، و پرسشنامه رضایتمندی برای دو گروه تکمیل شد. دادهها با آزمون آنوای چند متغیره سه طرفه آنالیز شدند.
یافتهها:
نمره کیفیت زندگی مادران دارای کودک اوتیستیک نسبت به مادران دارای کودک غیر اوتیستیک بهطرز معناداری پایینتر بود (P<0/001). اما، تفاوت معنادار در نمره اضطراب (P=0/114) و میزان رضایتمندی (P=0/079) دو گروه دیده نشد. عوامل سن، تحصیلات و شغل مادران بر روی نتایج اثر معنادار داشتند (P<0/05). همچنین، مادرانی که کودک مبتلا به طیف اوتیسم آنها زیر 1 ماه خدمات توانبخشی دریافت کرده بودند نسبت به آنهایی که بالای 6 ماه توانبخشی گرفته بودند، کیفیت زندگی پایینتر (P=0/003) و سطح اضطراب بالاتری (P=0/025) داشتند.
نتیجهگیری:
مادران میانسال خانهدار دارای کودک مبتلا به طیف اوتیسم پایین ترین میانگین نمره کیفیت زندگی را در بین تمام مادران مورد مطالعه داشتند. رضایتمندی از خدمات توانبخشی در مادران دارای تحصیلات دانشگاهی نسبت به مادران غیر دانشگاهی کمتر بود که نشاندهنده سطح بالاتر توقعات آنها از اقدامات توانبخشی است.
پیامدهایعملی:
کاربرد یافتههای پژوهش حاضر در مدیریت خدمات توانبخشی کودکان مبتلا به طیف اوتیسم و خانوادههای آنها.

Oral health is one of the most important issues for people and physicians, and it is very important to identify the factors that contribute to the damage to oral health. One of the factors that is always emphasized in harming oral health is drug addiction.
Methods:
This study is a systematic review and meta-analysis. We searched the electronic databases, including PubMed, Scopus, ProQuest, Google Scholar, and ISI for Persian and English articles and compared oral health indicators in patients with substance abuse with healthy subjects. The quality of the selected studies was measured by the Newcastle-Ottawa Scale. Heterogeneity of studies was performed using the Q test and I-square index. In case of heterogeneity of studies, a random effect model was used to combine the results. Publication bias was performed by funnel curves and Egger’s and Begg’s tests.
ResultsSubstance abuse had a significant effect on (std dif in means 1/657[1.873-1/442], P<0/001) and it has a strong positive and significant effect on plaque index (OR. 1/42; 95% CI 1/18-1/7), P= 0/0002.
Conclusion:
The mean DMFT was higher in people with drug abuse than in healthy people. Periodontal problems are also more common in people with drug abuse than in healthy people. Hence, the oral health status of these people needs more attention.
Practical Implications:
Responsible organizations and social dentists should pay more attention to oral and dental health of substance abusers.

Road Traffic Accidents (RTAs) are one of the essential causes of mortality;therefore, it is crucial to investigate the influencing factors. Studies indicate that drivers with attention deficit disorder have weaker driving performance than others. Therefore, it is vital to evaluate their driving performance in various circumstances, given their distinctive driving patterns.
MethodsIn this methodological study, a total of 62 individuals in both genders, aged 18 to 59, 31 with Attention Deficit Hyperactivity Disorder (ADHD) and 31 in a control group were randomly referred to the Sahand driving simulator or take the tests of Vienna Test System (VTS),after a definitive diagnosis by a psychiatrist . This study protocol aims to compare the driving performance of ADHD drivers with that of control group drivers. In addition, a joint investigation of driving simulator assessments and the Vienna Test System (VTS) including Reaction time (RT) and Adaptive Tachistoscopic Traffic Perception Test (ATAVT) will be performed.

Glial cells release different gliotransmitters and response to neurotransmitters released from neurons. These cells especially astrocytes, having different transporters, play an important role in synaptic space homeostasis and synaptic plasticity. In this study, the role of hippocampal glial glutamate transporter (EAAT2) in baseline synaptic response and short term synaptic plasticity were investigated.
MethodsIn this experimental study, ceftriaxone, EAAT2 activator (0.5mmol/0.5μl), was microinjected intrahippcampally for activation of hippocampal glial glutamate transporter in male wistar rats. Baseline synaptic response and short term synaptic plasticity were evaluated by field potential recording. fEPSP was recorded from CA1 following Schaffer collaterals stimulation. After Input/Output curve construction, short term synaptic plasticity was induced by paired pulse stimulations.
ResultsActivation of EAAT2 by ceftriaxone microinjection in CA1 did not have any effect on baseline synaptic response (P> 0.05, Two Way ANOVA). There was no significant difference in Paired Pulse Index at 20, 80, and 200 ms inter-pulse interval between ceftriaxone treated and control group (P> 0.05, Two Way ANOVA).
ConclusionThe results suggest that hippocampal glial glutamate transporter activation does not have effect on baseline synaptic response and short term synaptic plasticity in CA1 area of the hippocampus.
Practical implicationsConsidering the role of glial cells in regulating the excitability of the nervous system as well as synaptic plasticity, correcting these features of the nervous system by manipulating glial cells can help the treatment or prevention of neurological diseases. In this study, the role of glial cells in the homeostasis of the glutamate in the synaptic space of the hippocampus was evaluated, through the stimulation of its uptake, on the basic synaptic activity and short-term synaptic plasticity.

Attention deficit hyperactivity disorder (ADHD) is one of the most common psychiatric disorders starting at an early age with symptoms of attention deficit hyperactivity disorder and impulsivity. This study aimed to investigate the molecular alterations of the DRD4 gene, which have been linked to ADHD in previous studies with respect to the dose of methylphenidate.
MethodsThe study population included all ADHD patients needing drug therapy aged 5-18 years referred to Bozorgmehr psychiatric clinic in Tabriz in 2017 for one year. After diagnosis, symptoms severity was assessed using the Conners Scale for ADHD Assessment. Then, 4 ml of blood was collected from each individual and DNA was extracted using salting out method. After performing polymerase chain reaction (PCR), the products were separated using specific restriction enzymes and then polymorphisms of Ex3 VNTR and rs3758653 of DRD4 gene were analyzed in 2% agarose gel. The last dose to achieve the therapeutic response was recorded in mg/kg. Then, we investigated the relationship between the doses of drug used to achieve the therapeutic response and the existence of the mentioned polymorphisms.
ResultsThere was no significant relationship between treatment response and type of polymorphism. Also, there was no significant relationship between the average dose of drug and the polymorphisms of three replicates, four replicates, six replicates, and more than six replicates in EX3VNTR in DRD4 gene. In addition, we witnessed no statistically significant relationship between the mean drug dose and CC, TC, and TT genotypes in DRD4rs3758653.

Practical ImplicationsDetermining the reduction of ADHD symptoms by taking methylphenidate and evaluating the relationship between presence or absence of DRD4 polymorphisms and the dosage of drug to achieve a better response to treatment can collect enough evidence to determine the dose of methylphenidate according to the molecular analysis of DRD4 gene in patients with ADHD. This can reduce doctor visits, save time, and decrease the drug side effects.

COVID-19 disease is the cause of daily morbidity and mortality worldwide due to its high transmissibility and pathogenicity. To date, intravenous immunoglobulin (IVIg) injection has been used as one of the various treatments for this disease. This study aimed to determine the effect of IVIg injection on the treatment of patients with Coronavirus-induced cytokine storm.
MethodsA total of 174 patients with COVID-19 were included in this study based on their clinical characteristics and laboratory findings and were divided into two groups of IVIg recipients and non-recipients according to the treatment they received. Statistical analysis was performed using SPSS version 22.0. A p-value less than 0.05 was considered statistically significant.
ResultsIVIg was used to treat merely 20 patients and Kaletra and hemoperfusion drugs were used more among IVIg recipient patients (P=0.003 and P=0.001, respectively). COVID-19-positive PCR tests were significantly more frequent among IVIg recipients (P=0.026). The polymorphonuclear neutrophil (PMN) count (P=0.007) and sodium level (P=0.007) were significantly higher in the IVIg recipient group on the first admission day. Moreover, INR levels in the IVIg recipient group were significantly lower on the seventh admission day (P=0.020). The median of total intensive care unit (ICU) duration of hospitalization among IVIg recipients was significantly higher (P=0.001).
ConclusionIt seems that the use of IVIg in COVID-19 patients should be further investigated.
Practical ImplicationsIVIg injection could decrease mortality and slightly increase the survival rate among COVID-19 patients.

Children's meningitis is associated with many complications and deaths. Therefore, it is critical to develop reliable and available methods to predict these complications, diagnose them in a timely manner, and prevent the progression of complications. This study aimed to investigate the findings of computed tomography (CT) scan in patients with meningitis and evaluate its association with prognosis.
MethodsIn this cross-sectional analytical study, we investigated the clinical files of 100 patients with meningitis (preferably bacterial meningitis) in Tabriz Children's Hospital, Iran between 2013-2017. The results of CT scan and disease complications were recorded and analyzed by statistical software.
ResultsCommon symptoms identified in the patients included fever (88%), nausea and vomiting (36%), headache (27%), and convulsions (25%). Also, the results of CT scan included cerebral effusion (16%), hydrocephalus (7%), diffuse cerebral edema (7%), and cerebral atrophy (4%). Statistical analysis indicated a statistically significant association between the complications of meningitis (learning disorder, epilepsy, and hearing disorder) and the results of CT scan (cerebral effusion, diffuse cerebral edema, and hydrocephalus).
ConclusionCT scan can be used as an early diagnostic and prognostic method in children with meningitis.
Practical ImplicationsMeningitis is associated with many complications and deaths, and the lack of diagnostic methods and accurate prognosis is one of the important challenges in this field. Therefore, identifying and introducing accurate and cheap methods for predicting these complications and taking early measures to prevent the development of complications and treatment can be beneficial. The results of the present study showed that CT scan can be used as an early diagnostic and prognostic method in children with meningitis.

Premature birth, in addition to causing health problems in infants, increases stress in the mother and reduces attachment between mother and baby. Therefore, the present study was conducted to determine the effect of guided visualization on stress and attachment of mothers of preterm infants in the neonatal intensive care unit (NICU).
MethodsThis quasi-experimental study was performed on 35 mothers with premature infants admitted to the NICU of Mahdieh Hospital of Shahid Beheshti University of Medical Sciences, Tehran, Iran. Mothers were selected by available sampling method and randomly assigned into intervention and control groups. For mothers in the intervention group, the intervention was performed from the first to the fifth day of birth through training and sending guided imagery files. On the day of admission and discharge, the parental stress questionnaire and the postpartum attachment questionnaire were completed by the mother. SPSS software (version 20) was used to analyze the data.
ResultsThe mean total score of maternal attachment on the fifth day of the intervention was significantly higher in the intervention group (P<0.001). Also, the mean total score of mothers' attachment status on the day of discharge was significantly higher in the intervention group (P<0.001).
ConclusionImplementation of guided visualization program reduced stress in mothers with premature infants admitted to the NICU, and consequently increased the level of attachment.
Practical ImplicationsMothers are the primary caregivers of infants, and promoting maternal health has an effect on infants. In addition to reducing stress in preterm infants, this study can help improve the quality of infant care and increase attachment and parenting. The present study offers a completely cost-effective way to reduce the costs of long-term hospitalization and improve the quality of nursing.

Mothers with autistic children suffer a lot of psychological and physical pressures that can affect their quality of life, emotional states, and judgment about medical services. This study aimed to evaluate the quality of life, anxiety, and satisfaction with rehabilitation services for mothers of children with autism spectrum disorders and of non-autistic children in Ahvaz-Iran.
MethodsIn this cross-sectional descriptive-analytical study, 128 mothers with autistic children and 30 mothers with non-autistic children needing rehabilitation interventions were selected from Ahvaz rehabilitation centers. All participants were asked to complete Quality of Life (WHOQOL-BREIF) questionnaire, 14-item Perceived Stress Scale as well as Satisfaction Questionnaire. Data analysis was performed using a multivariate three-way ANOVA test.
ResultsIt was found that mothers of autistic children had a significantly lower quality of life score than those without autistic children (P<0.001). However, the two groups had no significant difference in anxiety score (P=0.114) and satisfaction (P=0.079). Factors such as age, education, and occupation of mothers significantly affected the results (P<0.05). Moreover, mothers whose autistic children received rehabilitation services under one month had lower quality of life (P=0.003) and a higher level of anxiety (P=0.025) than those who received rehabilitation over six months.
ConclusionMiddle-aged housewives with autistic children had the lowest mean quality of life score among all mothers studied. Mothers with a university education reported a lower level of satisfaction compared to mothers without a university education, indicating a higher level of expectations for their rehabilitation outcome.
Practical ImplicationsResearch findings can be applied in managing rehabilitation services for autistic children and their families.
